Who they are
Doom's power is that he is not wrong about very much. He is a better engineer than Richards by a hair, a genuinely capable ruler, and utterly certain that a world he does not run is a world being run badly. Casting the MCU's Tony Stark in the part makes that argument land differently — the face of the man who tried to put a suit of armour around the world, on the man who succeeds.

From the comics
Doom has worn the armour before
After Tony Stark is taken off the board, Victor von Doom decides to become Iron Man — sincerely, as penance, and to nobody's belief. A reformed Doom in Stark's role is an existing comics story, which makes casting the MCU's Tony Stark as Doom a structural joke rather than only a commercial one.

God Emperor Doom
When the multiverse collapses, Doom salvages the pieces into Battleworld and rules it as a god, with Doctor Strange as his sheriff. It is the definitive modern Doom story and the one an Avengers film called Secret Wars is named after.

Where this might be going
Guesses, not canon — reasoning from the comics and from what the films have already set up. None of it has happened on screen.
Battleworld, and Doom on the throne of it
Well signpostedAn Avengers film named Secret Wars, with Doom recast into the face of the Avenger who founded the saga, points hard at the 2015 story: the multiverse collapses, Doom salvages the survivors onto a patchwork world and rules it as a god. It would also let the MCU retire the multiverse premise and the Fox and Sony casts in one move.
Reasoning from the announced title and Hickman's Secret Wars
The Stark face is the story, not a shortcut
PlausibleThe cheapest reading is that Marvel wanted Downey back. The more interesting one is that a Doom wearing Tony Stark's face makes the argument Tony spent ten films losing — that the world would be safer if one sufficiently clever man simply ran it — and gives it to someone who wins.
